Dear Apoorva,
You have to fill Form D once a year or whenever there is a change in Basic or DA. Mention the category of worker and their common slab salary. When there is a change in the wage, you can update the same in the register. The importance of Form D is to show there is no discrimination between male and female workers in all cadres.

Dear Sir,
I am still confused about how to fill the Form D columns correctly:
1. Should each employee (both male and female) fill out the form individually?
2. If not, how should we indicate the Category of work for female workers in various roles such as Security, Production Engineering, Production, Design, etc.?
3. Whose allowances should be included in the Rate of Remuneration paid?
4. What amount should be listed as Basic wages? Should it be the average salary of all employees (both male and female separately)?
Please advise.
Thank you,
