Please consider your process and create your own designs. The designs should be based on two key factors: the convenience and usability for the user, and the support they can provide for the next person in the process, as well as for traceability.
Using standard forms may result in missing out on essential information, rendering the ISO system ineffective. It is advisable to develop your system based on ISO principles and pursue ISO 9001:2008 certification, rather than adjusting your system to align with ISO 9001:2008 manuals from other organizations.
As a practitioner of ISO and a member of the ISO certification body, I recommend this approach to you.
