There is no need to apply for voluntary coverage. Nowadays, the following forms are required to be filled up and submitted, and your establishment is covered by the authorities within one month.
There is a Business Establishment Registration form available with PF Office, which needs to be filled up. An undertaking from employees for voluntary coverage is supplied by PF Office, which also needs to be filled up. Annexures/documents asked for along with the business registration form are to be submitted.

I tries to PF office, hence they told that they dont have any format for Voluntary PF registration, so please any one send the attachment with the sample letter
