You have to fill out Form No. 2 in triplicate with the appropriate court fee stamp and submit the same to the Industrial Safety & Health Department. In the said form, you can enter the new name of the Occupier.

I am running a factory in Navi Mumbai M.I.D.C with 5 workers having a required power of 25 hp and holding all the necessary clearances and licenses from the pollution board, lube license, permanent SSI, and other required licenses. I even have shops and establishment license.
Now, a government department is requesting a FACTORY LICENSE. Please advise on what a factory license entails and whether we fall under the factory act.
Kindly provide me with your advice.

Since you are asking specifically about the change of OCCUPIER, you have to submit a letter mentioning the change of Occupier along with the memorandum of association or articles of association of the company wherein the change of occupier would have been effected by a special resolution passed by the board of directors. Mr. Vinayak's suggestion will be applicable only for the change of managers.
