I recommend that you prepare the draft on your own and upload it to this forum. This will help improve your drafting skills. If you receive a ready-made draft, it will be like spoon-feeding, which could hinder your growth. Just because someone assists you does not guarantee that the draft is error-free. Just send him a letter which outlines what his offenses are, for example, incorrect attendance procedure, and the provable misbehavior within the team.

Then tell him what the penalties are and what action management proposes to take if he ignores this warning.

Never tell people they are doing something wrong without telling them what the penalties are if they persist in their actions.

This is a simple letter and well within your capabilities as an HR person. You have no problems telling us, so tell him the same thing.
