Hi Tejesh,

I am working as an HR professional in a pharmacy company. Recently, I initiated an employee satisfaction survey within our organization and have created a survey form. Now, I am looking to distribute this survey to all staff members across India and need to include an attachment letter along with the form. Could you please provide me with the format for the attachment letter so that I can send it out to our staff?

As the sole HR representative in our organization, I am attaching the format for a simple satisfaction survey form.
