I think you should carefully read his terms & conditions of appointment and accordingly design the format. There is no special format as such for a Director as long as he's an employee drawing salary like anybody else. There should be appropriate components if he's entitled to a share of the profits on certain specific terms and if he is on the board of your firm. The attached format is for general use. If it suits your requirements, with modifications, you are free to use it.
