First of all you need to make different depramtnet n depending on that you can make the performance forms.
You can include the Following information inthe respective forms:
Employee Name:
Designation
Department:
Date of joining:
Due confirmation date:
Review date:( as in actually when was it done)
Name of the appraiser:
Designation of the appraiser:
at the bottom of all the pages:
signature of the Reporting Authority:
singnature of the Appraiser:
singnature of the appraisee :
signature of the HR:
