1. What documents need to be submitted to claim Maternity benefits and to whom should they be submitted?
The insured woman (IW) is required to submit the following documents to the Local Office/Branch Office - ESIC:
(i) Not earlier than 15 days before the expected date of confinement, a certificate of expected confinement in Form 18 along with a claim for maternity benefit in Form 19 stating the date on which the IW ceased or will cease to work for remuneration; and
(ii) Within 30 days of the date on which her confinement takes place, a claim for maternity benefit in Form 19 along with a certificate of confinement in Form 18 and a self-declaration pertaining to the details of her surviving children as of the date of presenting the maternity benefit claim.
2. What is the rate of Payment/How much do they provide? Is it equal to my in-hand salary or less than?
The rate of maternity benefit is roughly equal to the average daily wage. Maternity benefit is payable for 26 weeks to the IW with up to two surviving children and 12 weeks for subsequent surviving children, subject to payment of contributions for not less than 70 days in the immediately preceding one or two contribution periods. By way of the amendment in the ESIC Rules, an insured woman will be entitled to receive maternity benefits for 26 weeks, of which not more than 8 weeks shall precede the expected date of confinement. However, an insured woman with two or more surviving children will be entitled to receive maternity benefits for 12 weeks, of which not more than 6 weeks shall precede the expected date of confinement.
