I need evalution forms, in which the candidates communication, Attitude, Technical skills, softskills will be evaluated by the Interviewer.
In our company the interviewer orally gives the feedback to me.
So, I would like to make the interviewer to fill that form.
Which will be help full in documantation.

Here is a strong suggestion. Please make sure the Interview Feedback Form is just a page. It is psychological thing and the interviewers are more willing to fill them than the ones that run into number of pages.
I am working on a one page Interview feedback form will share it with you once done.
