First of all, it is largely a state subject (rules of Gujarat). Yet, the minimum in 9 days of national and festival holidays you have to declare. Normally, Form XVII speaks of details of salary. Buy the local Shops and Establishment Act book with rules as well as the state rules of the Factories Act. That would guide you better.
