Would there be a format for declaration/acknowledgement by employee at the time of relieving which mentions that he/she has completed the full and final settlement and that he will not disclose the confidential information to competitors etc.?
Also is it legally valid?

Sita Before relieving you should ask the person to sign on no dues left document so that it is clear that company is not liable to pay any kind of remuneration later. Regards, Allisha

There should be an exit interview for the employee who has resigned and going to leave that company after he hand overs all the documents and credentials etc. and in an exit interview you can get signed a document that mentioned about non-disclosure of credentials and client details or any other confidential things about the company.

Safe relieving is as important as appointment. Always keep in mind that, by relieving an Employee the Organisation is not going to make it's impression or reputation bad in front of the Employee.
But safety point of view you can mention the following:
Name of the Employee with compleet Address with contact details
Department
Designation
Since how long he has been associated with you
Reference of his resignation Letter with date
The date of relieving
Along with this you can give another letter showing has nature of work, last drawn salary, conduct and experience
But do not forget to take No Dues Certificate before signing the letters, which shows that his account is clear from every department of the Organisation.
As a precaution you also can take acknowledgement of the Employee on a Letter of Final Settlement, mentioning, he has received all his due and further there is no due to be paid. And it will be more batter if you pay the last final Amount by Cheque and mention the Cheque No and Date on that Letter of Final Settlement.

W.R.T the acknowledgement of Full & Final Settlement, I think it is a process which is processed by the organization. Taking acknowledgement & acceptance from the employee on the copy of F & F statement itself is an authentic document.
As far as Confidential Information is related, the employer should sign a Non-Disclosure Agreement which binds the employees with sharing the confidential information with any third parties, without the consent of the employer, during the course of employment & post resignation as well.
