Hi, I am raj and would like to know while filling up the form 10c and 19 do we have to mention the old PF prefix for example: KN/BN/24960 has changed to BG/BNG/24960. Since i left the organisation 5 years ago and i need to make an affidavit and if i make it on old number and Hr want the new number then the affidavit would be waste and also in need to submit both forms again. please do the needful by answering the question. thanking you with regards Raj

Hi Dasukh,
It seems from the a/c numbers you have mentioned that the Region and Sub-Region codes have been changed. In your claim form(s) you may write your a/c number under latest region and sub-region code and also mention the old region & sub-region codes in brackets.
