Hi Priya,
If you change the company, You are not given the same PF account number but you can transfer you PF contribution to new PF account, Which is being opened by your new company. You have two to ways to proceed....
1. You have to claim your PF contribution by submiting Form 10C and Form 19 then the amount will be transfred to the given bank account number.
2. If you don't want to claim, You have to transfer your PF contribution by submitting Form 13 A in the present (Changed / New) company.
New company will get Nomination / Declaration form from you and will get you the new PF a/c number then you need to submit Form 13 A to transfer your old PF ammount to the new Pf a/c number.

Priya,
You need to submit Form 10C and Form 19 in your old company to claim. If you have the good rapo with HR, you can courier the forms by filling require information and keep the HR informed through Phone.
