Industrial visit is a part of the Education, during which students visit companies and get insight of on the internal working environment of the company.
The industrial visit also provides an insight on how companies work and also useful information related to the practical aspects of the course which cannot be visualized in lectures.
The Questionnaire varies from industry to industry and so a sample format is attached for your convenience.

Whenever you share a bad experience avoid mentioning the name of the company, people in open forums like this. Hope you don't mind this piece of advise.
For all you know there might have been some lack of communication from your end that it is HR students who are coming and you would like to have such and such things to be covered during your visit.
Learn from this experience and plan your next visit accordingly.
In fact the brief guideline I gave was done in an urgency, as I saw your request just the evening prior to your visit.
