The design depends upon your objectives. May I know why you are asking demographic questions? If you have to seek personal information, it's better to do it at the end. You should also inform the respondents why you are seeking their help; this can improve the response rate by explaining the benefits of investing their time in answering.
I encourage you to put on your thinking cap and consider why you are using different scales, ensuring they are balanced and not skewed. Also, check for consistency in the tense of the words used in your scales.

I think demographic questions are asked to determine whether the person has the knowledge or aptitude to be considered for data collection. I will shift them to the end. What do you mean by scales? Are you referring to their balance or skewness?

Please compare the scales used for Q7 and Q11. The scale for Q11 is balanced as it has ratings for agree and 2 for disagree, whereas Q7 has 3 for agree and 1 for disagree. I termed the former as 'Balanced' and the latter as 'Skewed'.
