Your questionaire will be based on the training you have done in YOUR organisation.
We do not work for your organisation so we cannot devise the questions for you.
If you are looking at effectiveness of training overall, then you need to talk to the various divisions in your organisation and determine if the training has changed anything. Is the company more profitable from training staff to do jobs better or more efficiently? Has training improved overall loss of downtime due to accidents? Is there better morale and teamwork? Etc, Etc.
Only you can devise the questions that need to be asked, based on your particular organisation.
