Leave application to be filled, signed by respective employee who would like to proceed on leave and get the leaves balances verified from HR Department and get them noted on the leave application and then the employee got to submit to his immediate boss and later on approval of his immediate boss, it would be submitted to HR department, for taking leaves in to individual's account in leave register.

Hello dear.
According to me it is not HR duty. One major duty of Hr Dept. is to keep & maintain the records. You are supposed to collect the leave applications signed by the applicant who is applying for leave but not to sign yourself...
Hope you r now satisfied with my reply.
