Dear Chitra,
Hi. as I understand the subject the questionnaire that you are looking for will have to be made by you & there can not be any ready made solution. You will have to catch hold of their Job analysis, PIQ ( Position Information questionnaire ) and then see the linkages drawn to their PMS ( Performance Management System ) with the appropriate weightage drawn, if any, since their will be not any one but a set of competenies forming the JS ( Job Description ). This is a colossal task by any standards. To assist you in this I can provide you with some informative material on Competency Mapping, as attached.
