Dear Madhura Parab, Please contact the respective PT office with written request for loss of challan, you can get the duplicate copy from the CTO.

Hi, Do carry bank statement to PTO, which states that particular cheque issued towords pt contribution has been realised. They will verify and issue a duplicate pt challan. Regards, Prabhanjan

You have to prepare duplicate PT challan which you have lost and then contact your bank where you have paid they will autorised the same.

Pl. fill a duplicate copy of the Professioal Tax form and put cheuqe details , Cheque date everything and go to PT Office along with the challan. They will verify it & they will put seal and signature on the same challan.
