Earlier we have taken ESI code number from Delhi. our all staff is registered under Delhi ESI office. we have opened another location in gurgaon. 50% of staff has been shifted gurgaon from delhi.
now as we are operating in gurgaon, do we really need to take new code in gurgaon. if we take esi code in gurgaon also, will we need to fill new forms for every employee or the old form (already submitted in delhi) can be transferred to gurgaon office..

You neednot get Declaration forms for all such employees(gurgaon). You write to New delhi ESI RO with details such as:
1. about your new gurgaon office
2. that you are paying contributions from New delhi office for gurgaon branch
3.that you will submit seperate RCC(halfyearly return - return of Contribution Cards) directly to Newdelhi office under copy to Gurgaon ESI office.
If it is possible you please visit your delhi esi office as well g
urgaon esi office in person and get concurrance.
After that you can send your returns and remit the contributions in seperate challan from delhi itself.
Declaration forms for newly joined persons you can collect and submit in Gurgaon esi office.

If new code is alloted from Gurgaon ESI office what would be the procedure for taking the new code.?
CAN ESI CONTRIBUTION BE DEPOSITED FOR ALL UNITS ALL OVER INDIA FROM THE CORPORATE / HEAD OFFICE ? IF YES WHAT IS THE PROCEDURE.

The gurgaon office will allot an additional code for your gurgaon office - the employees insurance numbers will not get changed; they can use the same with change in Local office and Dispenssary.
if you transfer the gurgaon employees further to Noida,like that, you can (if it is short period,)issue form 105 for their medical benefit at the transferred place whereas the contributions are to be paid in the place originally allotted.
As already told, you can pay all the contributions in the place where head/corporate office situated but in seperate challans. The return of contributions are to be submitted to the respection regional / Local office from corporate /head office under copy to the regional office where the Head/corporate office located
