In 2011-2012 employees who were drawing more then Rs. 6500/- are wanting to opt out of pf. So old format of pf form 11 is required.

Dear member, Please note that an Employee already covered under EPF, will continue to be covered till he is in the Employment of the company notwithstanding that EPF Wages is above the coverage limit of Rs15,000/- However, you can restrict EPF Contributions to ceiling limit i.e Rs15,000/- (Rs1,800/-)
You can not exclude an Employee from the Purview of EPF Coverage with retrospective date.You can exclude an Employee from coverage at the time of Joining only based on information furnished in Declaration Form-New Form # 11
