In your questionnaire, you have given objective as well as subjective questions. Format is such that these have to be filled manually. Persons like me who don't have printer at home, cannot help you in your research.
I want to give you suggestion. Create form such that it can be downloaded and filled on computer itself. It can be uploaded back and mailed to you. It may help you get more responses.
