An ID card should have the institution logo or name in the first then the recent Photo of the Member below that

Blood Group: ( it will be very useful in case of emergency)
issuing authority sign
in the back side of the ID card institution address should be there in case if the Id card is lost they will return it to the concern address.

Why dont you approach a Design agency for this? They are good at it. I guess you can concentrate on other areas and leave this to the best people in the industry to do it.

Aside from the ID design and layout to be, I would like to have side comments about Company ID card.

IDENTIFICATION CARDS

On the first day of employment a photo-bearing identification card is issued by the HR Department to each employee. The card, which is used as a building and dining room pass as well as in cashing checks, is to be available for presentation during working hours or, in some cases, worn for ready identification.

All cards bear an expiration date for the end of the year. It is the responsibility of the department head to assure that a new tamper-proof tab with the new expiration date is placed on each card.

The card is the property of the Company. When an employee resigns it is, if possible, collected by the HR Department at the exit interview and replaced by a temporary card which expires on the last day of employment. The supervisor is asked, however, to ensure that the permanent card is collected and returned to the HR Department.

An employee reporting a lost card is issued Temporary Identification Card to use as a cafeteria pass and for cashing checks (checks must be initialed by an officer in the individual's department).

After seven business days have elapsed and the supervisor has determined that the card is lost. A replacement may be requested through a memorandum to the HR Department explaining the loss and signed by both employee and supervisor.

Consultants and temporary employees are provided cards each quarter for access to a Company building and its cafeteria. The HR Department supplies these cards to department heads who are responsible for their issue and collection. The cards bear expiration dates and are signed by the department head who has issued the card.

Identification cards must be shown to the guard on duty when entering Company buildings and individual departments when necessary; between 4:00 p.m. and 9:00 am. daily, holidays and weekends at general offices; and between 7:00 p.m. and 8:00 a.m. dally, holidays and weekends at management personnel offices, computer center and designing center.

Employees are issued Security Strip identification cards for access to the restricted buildings and security areas. These Security Strip identification cards are programmed for each employee according to the building or security areas to which he or she may require access. The Security Strip identification cards must be passed through readers mounted either on turnstiles or doors in order to gain access. Correcting the security clearance of a transferred employee is the responsibility of the new supervisor. Employees who require entry to security locations but do not have the necessary Security Strip identification cards must be identified at the entrance of the security area by the supervisor of that department.

The HR representative or Security administrator coordinates the distribution and revision of identification cards. Lost Security Strip cards must be reported immediately to the supervisor who notifies the HR representative. The external-number on the lost card is reported by telephone to the Security Center, and is temporarily deactivated. If the Security Strip card is not found within seven business days. Access Control Security class (ACSC) Input Transmittal Form is sent to the HR Department confirming that the card is lost and requesting a new card to be ordered. The HR Department arranges for a photograph and a new Security Strip identification card. If a card is found or returned to an employee, he or she must notify his or her HR representative so that the card may be reactivated in the Security Center.
