Atleast i can't forward the pay slip but i can advise it should be prepared in such way what is the salary accured and clearly showing what is deduction, number of days worked, what is EL, CL,SL and what is leave balance available, PF Number, ESI Number , ESI Discipinary
It should be slip which can be easily understood by even a layman
