When we are going to job satisfaction survey then it should be related with both I.e.nature and position of job. Job satisfaction questionnaire should be deal with everything which are related to perform these specific job I.e. task, duties,roles & responsibility, Organization culture, your immediate supervisor, compensation & benefits, employees engagement towards organization, employees value preposition etc.....It will be better if you make a topic in the name of " NATURE & POSITIONING OF JOB "

Grievance arises when an employee feels that something has happened or is going to happen which is unfair, unjust or inequitable.
It may relate to either one employee or group of employee. So that grievances questionnaire should be related with
- the employees feelings and opinions about the company policies and practices.i.e. questionnaire should be related to interpretation and implementation of agreements, labour legislation,various personnel policies, rules and regulations, past practice,code of conduct .
- behavior of his immediate supervisor to respond complaint
- behavior of his department head to respond his complaint
- behavior of grievance committee to respond his complaint
- behavior of Management
- role of Union.
- wages paid or not if concerned employees may go for enquiry during the working time.
- time taken to resolve the grievance is satisfactory or not
- communication I.e. decision taken to redress the grievance of the worker must be communicated to him.
-level of freedom of expression to worker in front of Management
- follow up action is efficient or not by his supervisor, department head, grievance committee , Management and Union and specially by personnel or hr department
You can setup a questionnaire based on above explained points.
