Hello Alok,
The purpose of sharing this warning letter - Whether for information or need correction you have not mentioned. How you came to know that employee had extended the leave till 1st -Aug-2019. Not clear However please refer the modified one as per your draft. In case if you have certified Model Standing Order or follow model standing order you can refer that clause here( serious misconduct as per our Standing.........). Advise the employee to sign the second copy of the letter as token of acceptance. In case of any previous warning you can refer the same here.

As requested by you, you were permited to go on leave from ____ to_____ and you were supposed to report back to office on____. This is as per your leave request as approved by ______.
However it has been brought to our notice that you have not reported to office on ____ and had extended your leave without any prior information to your immediate Superior /HOD. It is very unfortunate that a person of your stature who is supposed to be an example for your subordinates, extended your leave without any prior information. This had caused unnecessary hardship to your Team.
The above Act of yours is a serious misconduct as per the rules & regulations applicable to this Organization. You are hereby SEVERELY WARNED for your above act of leave of absence without any prior intimation and proper planning. You are hereby directed to refrain from proceeding on leave /extending you leave unauthorizely henceforth.
Your attendance and leave pattern will be monitored very closely and you are hereby warned to realign your leave pattern failing which disciplinary action will be initiated against you.
