State Rules & Forms under the Factories Act, 1948 differs from state to state, So it is important to know which state you are talking about.
For your information, under The Gujarat Factories Rules, 1963, Form 4A is "Form of Certificate of Competency issued to a person or an institution in pursuance to rule 2(a) made u/s 2(CA).
