Hi Swati,
First try to understand the different categories of work force in the organisation where ever you are doing the project and find out their role and responsibilities, KRAs( Key Result Areas) and get a feed back from them on the gap between their achievements and targets. Then discuss with each dept head/ supervisors & employees themselves on the reasons for the gaps i.e. whether they are lacking the required knowledge/skills/attitude to perform or they have some other physical and mechanical problems.This I feel is the most practical approach. However for forms and other material you can refer the site mentioned by other friends.
