U can decide the various common parametres which r generally required for all posts like, communication skills, presentation skills, technical skills etc. & then rate the candidate as per his skill levels!

Dear Nalini,
The format you have sent is fine and contains most of the attributes for assessing suitability of a candidate during an interview.
However, it would be a better idea to have one sheet for each candidate. One sheet is always easier to preserve in the personal file.

Well this side SANDEEP SINGH SASAN, REGIONAL HEAD HR NORTH from Infra Industry need a help that if any one has IAS form alongwith Skill set rating like for Mech. Er., Electrical Er., Civil Er., Technician, Mechanic, Operators, etc.
are you help me out to prepare new format so that it is going easy for us to select a guy.
