Good day !!!
As per the our annual appraisal session , we are going to conduct appraisals in our organization , so for the same i have drafted a appraisal form , but my employer want very brief and specific appraisal form .
for the reference i have enclosed drafted appraisal form , kindly advice .

Performance Appraisal system is as far my knowledge goes, setting up your Unit goal and dissolve / disintegrate into small set goals / to individual goal to achieve the unit goal, at the beginning of the year, and made known to the Appraise with ack. The Apprise review it every quarterly, himself. At the year end,the Appraise performance is Appraised by the immediate Boss, and with any opinion / hurdles faced by the Appraise in achieving the set goal. Finally by the Unit head, with endorsement / or non endorsement with differential views.
Carrier Appraisal is also on any contribution / achievement over and above the set goal, however depends up the slot vacancy on proposed carrier progression.

This is a good format. Rating in numerical is more effective. Weitage to every point to be earlier decided based on job profile.
