1. Facilities Manager
2. Accountant
3. Contract Manager
4. Billing Officer
Please, I need help in preparing these forms. May I have samples of performance appraisal forms for these designations?

Performance Appraisal (PA) forms do not vary according to the designation. The PA form is uniform across all designations. At best, you can have one form for managers and another form for non-managers.
The PA form is part of your Policy on Performance Appraisal. This policy itself supplements the Policy on Performance Management System (PMS). Therefore, first, you need to establish a well-designed policy on PMS. Apart from the Policy on PMS, you need to train your staff on designing KPIs/KRAs or differentiating between these two. I recommend that you do this spadework first. Any half-hearted job or piecemeal work could do more damage than good.
I provide consulting services on establishing a comprehensive PMS. I study the systems process of each department, find out the direct and indirect costs of each department, and design KRAs that are centric to these costs.
