Hi, Pls guide me. Whether Form 12B is filled up by employer or employee. Previous employer is concerned about only form 16 right.

It has to fill by employer not employee.
It will issue when employer has paid perquisites to employee. Even If employee doesn't have any perquisites employer can issue Nil form.

The Form 12BA must be issued by the employer after remitting the TDS to government. If required the employer will give NIL form also.
But you can have a check through your bank account also if you have registered for net banking in ICICI bank or in incometaxdepartment website by log in your PAN No.
