Few guidelines on drafting.
1) Devote first few lines on why discipline is important for productivity and achieving organisatioal goals.
2)Alott next few lines how the acts exhibiting the kinds of conduct described in your post amount to breach of discipline and jeopardise the production and safety of employees.
3)Earmark few sentences on advising the employees to refrain from the prohibited conduct.
4) devote last few lines bringing to the notice of the employee the consequences of breach of discipline i.e committing the acts of conduct enumerated in your post.
Now yu can draft a notice on your own.
