Every firm has the responsibility to provide a safe working environment for all of its employees and will not knowingly subject employees to unsafe working conditions. In addition, employees should take reasonable precautions to protect themselves in the performance of their jobs. This applies to both company and customer facilities. Company has the responsibility to inform employees of all known potential hazards in their works environment and the precautions currently being taken to minimize hazards. Safety procedures will be established in all instances, where necessary, and well. Accident prevention, provision of a safe working environment and property protection are the responsibility of every manger, supervisor and employee.
Top management's support is manifested through company policy: formal, definitive and published, thus stating management's acceptance of its responsibilities.
Each host manager is responsible for safety and loss prevention at his facilities. Prevention of accidents and injuries is accomplished through control of the work environment and human activities in the workplace.
Employee has responsibility as well, each employee's acceptance of his responsibility to protect himself and his fellows. This determines the success of all company Safety and Loss Prevention efforts. Each worker expends the most efforts toward those aspects of his work which he perceives his superiors to be induging him. Where there is emphasis on safety by supervisor, and his managere must be held accountable for his actions.

My comments on your post:
Top management is the prime responsible for the any accident prevention program. Their committment matters for any begining. Second thing is that once the top is committed all rules and procedures and system will come in place for accident prevention. Furthse need based training will be imparted upto the workers.
Even though all these done in my opinion the main role of any accident prevention program lay on with the first line supervisors. The key person is the one who executing the job.

The key prson in preventing accident is the person himself.Safety Officer or other officials can only give guide lines to implement safety policies.But they can't be omnipresent.Sometimes the company is not serious about workers safety and does not care to equip the worker with proper safety gears (PPEs) Under such condition a person can still prevent accident if he has a proper attitude towards safety,if he thinks that "safety is everybody's responsibility,Espectially Mine",if he is concious towards his nvironment,removes all unsafe conditions before starting work and implements all controls to avoid the accident

The key person in preventing Accident/Incident is the person carrying out the activity. Everyone in the organisation, whatever it may be the level, hold the key of Safety. Management is Responsible to creave an avenue to work safely and everyone who is performing his duty is accountable for completing his task safely.
Responsiblilty without Accountability is a pleasure trip.
Safety works well where top down approach is being implemented effectively. Feed back and suggestions to improve has to go from Bottom to TOP.

Fundamental tenet of safety states that the supervisor is the key in accident prevention. This seems axiomatic in our thinking. The supervisor is the person between the management and the workers who translate management’s policy into action. The supervisor has eye contact with the workers.
Is this the key person? In a way, yes. However, although the supervisor is the key to safety, management has a firm hold on the keychain. It is only when management takes the key in hand and does something with it that key becomes useful. Safety personnel have sometimes used the key-person principle to focus their efforts on frontline supervision, forgetting that the supervisor will do what the boss wants, not what the safety specialist preaches.
Management must delegate or assign responsibility for safety down the line, but without losing any of that responsibility itself. This is where many safety programs fall apart: responsibility is assigned to the line organization, but no more thought is given it by management (until the record turns bad, perhaps). Yet to assign responsibility to the line without fixing accountability is meaningless. This is exactly what we had done over 70 years in safety, and in many cases we still do it today.
Supervisor is the key but the key person is the one on top management who holds the key and who has absolute responsibility with regard to safety.
