Fill up and sign some forms to apply for gratuity or if the onus is on the employer to do it?

It is more than 50 days since i got relieved from my company and i had completed 4yrs 8 months and 20 days. my accounts manager said gratuity has been applied and he is awaiting it from lic. I need to understand whether i have to fill up and sign some forms to apply for gratuity or if the onus is on the employer to do it?

Dear Raju,
It is onus of your company to clear the gratuity dues which supposed to be cleared within 30 days. Please check with your company as for the delay there is penalty applicable on the gratuity amount.
