Dear Piyali,
You can directly approach to the HR Dept. in your company for this case. There are two forms for withdrawal of PF. Form no.10C for Pension Scheme & Form No. 19C for Provident Fund. I have also attached these forms for your reference.

Dear Piyali,
What Mr. Vineet stated are correct and applicable for claims claimed with in one year of leaving the services.
If the period exceeds more than a year then you have to submit the following-
-PF Claim forms (F19 and F10c)
-An indemnity Bond
-Copy of Your PAN Card or Voter ID
-Bank Pass Book First page xerox,
-Relieving letter issued by your previous employer mentioned your PF Number.
