I will suggest you to please read/go through the procedure for such claims of maternity benefit as laid down in the Maternity Benefit Rules as framed by the appropriate government in the jurisdiction of which State the Hospital in which you are working is located. In order to submit your claims you are required to comply with the said rules and follow the procedure as laid down in said rules. You can find such rules either in any law bookshop in your city or even online from the website of Labour Department of the concerned State Govt. of the State in which said Hospital is located. You can also get information regarding such rules and procedure from the appropriate office of Labour Department.
2. I may however, also inform you that the compliance position of labour laws in this country is very very poor. The employers devise various methods to exclude its employees from various benefits under various labour laws. To engage the persons on contract basis, or as a consultant or professional etc.(and continue their services for long periods and getting same type of work as that of their regular staff )are such methods vide which such benefits like maternity benefit, gratuity etc. are denied to employees. In addition, the employers or their contractors generally terminate and discontinue the services of such employees ( on one pretext or the other) who claim such benefits from them. I am not discouraging you by writing above words, but I will certainly like to express my apprehensions.
