Hello Priya,
First of all, the ideal way to be relieved from an organization is to submit the resignation in the proper manner and complete the formalities as per the company's procedures. In your case, you have not mentioned at what level you are working in the company and what the actual reason for your relieving is. It all depends on the kind of relationship one maintains with the boss and other senior officials.
a) If you really want to quit for medical reasons, you can call and talk to them over the phone and explain the problem. Additionally, you can also send an email to your boss, cc'ing HR and your senior bosses.
b) If you are planning to take up another job, the best approach is to go in person and explain that you have personal and family reasons and are unable to continue your current job. Request to be relieved at the earliest possible.
c) If you have any other official problems in your office, you have to address the issue yourself based on merit and request relieving.
So, think carefully and make a well-informed decision. Any company would expect the outgoing employee to separate in a smooth manner, transfer knowledge to the incoming person, handover responsibilities properly, complete all relieving formalities, and more. It doesn't end there. Ensure all your dues are settled correctly, such as gratuity, leave salary, performance incentives, subject to your eligibility.
Hope this information is helpful to you.

Dear ALL,
This site is for guiding people who do not know HR policies, procedures, etc. I hope we need not spend time on this kind of people. I mean, we should respect the site HR. Applied for sick leave and now wants to join a better job by providing wrong information. No proper handing over of documents, materials, etc., given by the previous company. Leave aside laws, it is unethical and immoral.

Dear Mr. Anonymous,
This is not a proper way to get separated from the company. You are wrong in your approach with whatever you have done without any logic. The company is right from its point of view. Now, you are just in need of F & F dues and relieving so that you can show in your next company which you have already joined.
For a smooth transition, simply reply to the email attaching all your medical bills and reports so that your incapacities to join your company for a longer period can be considered, and you can be relieved on medical grounds.

Dear Madam,
Your company is right; you must have received an appointment letter wherein the terms and conditions of employment with the company were outlined. It is akin to an employment agreement, and the parties involved must adhere to the terms of the appointment. Proper handover of documents, one month's notice, or salary in lieu thereof is required.

Dear X,
This is not a lawful resignation. You should be professional in this regard. This is not a school or college where you can leave whenever you want. This is a corporate environment, so you should adhere to its policies and procedures.
Thank you.
