I have worked for Organization 1 from Nov 1999 to Jun 2004 and then for Organization 2 from July 2004 to Sep 2006. Since Oct 2006, I have been working at Organization 3 and continue to work there. My EPF has been transferred from Organization 1 and Organization 2 to Organization 3 and is reflecting in the EPFO Passbook. However, my EPS does not reflect in the passbook. I had followed up with EPFO for Organization 1 and Organization 2 and have obtained a copy of Annexure K, which shows the amount of EPS contributions.
I am now considering applying for Scheme Certificates. However, Organization 1 has merged and been acquired by a new company, so that legal entity no longer exists. The same situation applies to Organization 2. I have the following questions:
1. Can I fill out Form 10C, obtain attestation from a Bank Manager, and directly apply to the respective EPFO for the issuance of a scheme certificate? If yes, are there any other documents I need to attach along with Form 10C?
2. Will Annexure K help in any way to prove my service period, as it contains all the records of Pension Contributions during my tenure at Organization 1 and Organization 2?
3. If EPFO returns the form stating it needs to be endorsed by the previous employer, is there any other recourse?
