Thank you for posting the questionnaire and seeking feedback rather than asking for one on-a-plate. My comments and suggestions are:

1. It's better to ask personal questions at the end. Please search the web for links on questionnaire design.

2. You need to include a covering letter or give an introduction as to why you are conducting the survey, how it benefits the respondents, and how they should respond to each of the questions. For example, should they tick boxes, circle the options or underline them; slect one or more options, rank them in the order of their preference, etc.

3. You may consider asking respondents to rank the choices in descending order of importance.

4. Q4 has two aspects: suppose the interview was well co-ordinated but not conducted, how should the respond and how will you know which one was deficient? Same goes for Q6 and other questions where more than one factor is included.

5. For Q8 and Q10 a different scale such as the one used for Q11may be more suitable.

As I do not know what course you are pursuing or what's the objetive of the study and what hypotheses you have formulated, it is difficult to comment on the adequacy of the demographic data that you are collecting. I would suggest that you collect information on the age, sex and previous experience of the respondents.
