First of all, please tell me, whether you have a Performance Management System established in your company. PMS, as it is called, should have been one of your company's policies and it is a complete approach to aligning the company's vision, mission and objectives of the company. The entire organisation need to be mapped for this process, procedures established, all the stake-holders involved in the process. We cannot just give you some formats and templates, as these are also designed as part of PMS.
