For Labour Contractor license,
He should get an agreement copy from the Employee that he is engaged in his factory,
he should apply for Pf and Esi License,
He should get form from inspector of factories and attach the agreement copy obtained from the employee.

As already replied for getting contract Labour licence we are required to go through the applicable Central or State Rules to know the procedure for licence. Therefore first examine which of the Rules are applicable in your case & then go through them.
