Pls help me to provide for format of warning letter for irregular attendance

Please find below the draft of letter:-
Dear Sir/Madam,
We would like to inform you that management has noticed that in last three months you have taken leaves (full day or short leave) mostly without any prior intimation to the company & it seems a regular practice.
In further you may please note that no half day/ full day leave will be given unless you informed the management atleast three days before availing any leave.
Kindly treat it as a first warning.
Hope it will serve your purpose.

Please find the attached warning letter for not coming regularly:
If the employee is not coming regularly,before giving him warning letter explain to him orally.If he continues the same,then plan for warning letter.You can ask the concerned dept heads or supervisor of that dept to take care of that issue and can ask to levy penalty.

Hi Gaurav,
I think you can prepare a letter by mentioning two things,
1) You are on uninformed leave from such and such a date
2) If you don't report back to concerned person within ---- hours than your services will be terminated
Mention this things and similarly if he doesn't turn back than terminate him from the company.
I hope this will solve your problem.

Dear (name):
Your work has been excellent. However, your poor attendance record is now overshadowing your work record. I realize that your health has been poor but one of our job requirements is regular attendance. We have difficulty scheduling when we cannot rely on employees due to frequent absenteeism.
This subject has been discussed with you on several occasions. Now your attendance must improve to meet our requirements or we will have to take further disciplinary action, possibly including termination.

I am working as a HR in a private company,Kindly give your suggestion on following matter, A fresher joined in our company 3 months before.
