You can have an objective of the study you are doing and what information you require from the respondents, you can frame the questionnairre. because you cannot get a standard questionnairre, firstly, you should be clear what information you want.
once you decide the information you require, you can beautifully make the questionnairre.

thanks. all i want to know is that since my project is going to be in a small scale mechanical industry, and most of the employees there are educated only till sslc and some till puc, what should be the type of language used in the questionnaire to enable maximum understanding and minimum room for confusion?
