Can we take the written feedback on manager from his team members to assess his/her performance or attitudes.

Hi Sundar,
You can always do that once you incite the confidence among the team members that you would keep confidentiality. You may ask them to fill the form without mentioning their personal details. Also, you should ask them not to discuss among themselves about the appraisal they have made.
Your questions also must be framed in a professional manner.

You can take feedback in the form of a well structured questioner. You cannot just ask subordinates to write a small note on their manager. That will amount to “work-place Politics”.
