There is no statutory format for break certificate or a certificate of non contributory days. In the olden days when we used to send the form 3A in respect of each members a form used to be attached in support of non contributory days. When everything became online, there is no need of such a statement. But sometimes the EPFO will reject the application for pension for want of this statement. I think you may prepare one in the attached format and submit it. This should solve the issue.
