Firstly who is the employee and who is the sanctioning authority?
What is a big deal in drafting a mail for yourself on this?
Leave application should be generally through a hard copy submitted with the recommendation to sanction by his superior and then HOD sanctions it.
Even if it on email request can be made with the content and dates, that is enough.
