Followings are the steps to take printout of submitted accident report.
1.Logon to your ESIC portal--Accident report (form 12)
2.Insert IP number
3.Select Modify Last Report in Do you want to selection Column
4.Click Submit
5.You will get Currently modification approval is not provided error message click on OK
6.You will get IP Details again Click on cancel
7.You will get Benefits Upload Certificates
8.Select Accident Report Mapping
9.Fill up the insurance number
10.Search
11.Again Search
12.You will get Checkbox to map Accident report
13.Select Reference Number
14.Print Accident report
