Dear Sunil It’s very simple just go to ESIC Portal fill the accident report & submit to ESIC or (Nearest Police Station/DLC In case of death)

You need to first submit the registration of IP online, obtain the insurance number. Then follow what Vinay.HR said.
Onus lies on you to prove that the accident occurred on very first day of employment.

1. Accident Report on prescribed form is required to be submitted to the appropriate Branch Office of ESIC even if the Insurance Number has not been allotted. Delay is always difficult to justify later on. You can submit the Declaration Form of injured employee online or by old procedure as the Branch Office may accept and on Accident Report in the column of Insurance Number of employee, you can indicate as Applied For.
2. I also hope, that you will also make compliance in respect of other issues/points- viz. entry in the Accident Register, intimation of accident to the Inspector of Factories and in case of serious accident a Report to the appropriate Police Authorities (FIR).

I think, since the accident report is now introduced online in the portal, there is no need to submit it to Branch office in physical form. But in given case, the queriest can submit the physical prescribed form of accident report to the Branch office without insurance number. Still I prefer to enroll the person as IP first and submit the accident report online. This will not take much time. Only thing, one need to have basic details of the person to whom he is going to enroll.
